• Recertification: You will receive a renewal reminder approximately forty-five 45 days in advance of your certification expiration date.
• Choose a recertification location from the list provided on the Renewal Form. On the day of recertification you MUST bring to the test site the following: $30 recertification fee; Two (2) forms of identification; and your Renewal Form (signed by your current or previous employer).

Practical test cannot be taken on Computer or by Paper and Pencil.
