The fastest option is to send an email with your name and the last 4 digits of your social security number to fdnyschedule@psionline.com. Your duplicate notice will be emailed to you within 24 hours. If you do not have an email address, you can call 1-844-774-8378 and leave a message with your physical mailing address, a phone number and your name and social security number.
